The US opioid epidemic, related to legal prescription drugs in a specific geopolitical context, has health outcomes which differ from those of cannabis or other drugs.

They note that there are "forms of regulation that do not seem to contradict market prohibitions", including decriminalisation and regulation of opioid substitution therapies. They also argue that there is "not sufficient evidence supporting that countries should further legalise cannabis or other drugs".

However, only a few sentences later the authors downplay the importance of smoking: "risk factors unrelated to tobacco are increasingly responsible for the burden of COPD, and are likely to surpass the risk attributable to smoking within the next two decades".
